Combine all ingredients in a small sauce pan.

Simmer on medium heat until thickened.
This is definitely not keto friendly! I haven't tried this recipe with a sugar alternative to reduce the overall carbs. It probably would work with some adjustments like replacing the cornstarch with xantham gum. I will try this soon!
Tips for Success
Mix cornstarch and cold water first to avoid clumping and then add the other ingredients.
Adjust sugar to your liking. I used Heinz No Added Sugar ketchup. If you use 'regular' ketchup, you might want to reduce the sugar even more.
